1. The work. For the inspection or crash above, Truck Service Hub LLC ("TSH") will:
- prepare the Request for Data Review from the inspection report or police crash report and your records, with supporting documents attached;
- either submit it in FMCSA's DataQs system from TSH's own DataQs account on your behalf, or give it to you ready to submit from your own DataQs account. TSH never asks for your FMCSA, Portal or DataQs login;
- answer any request from the reviewing agency for more information, and send you each decision;
- for an inspection, if the state does not correct the record and there is a reasonable basis, take the request to the next review stages (Reconsideration, then Final Review) at no extra charge.
2. Authorization. By signing, you authorize TSH to prepare and submit this one Request for Data Review in DataQs on the carrier's behalf, and to communicate with FMCSA and the reviewing state agency about it.
3. Fee. $199 flat, charged when TSH accepts your case, before filing. For an inspection, it covers every violation listed on that one inspection, through every review stage. For a crash, it covers one request on that one crash. The fee is the same whatever the decision. There is no success fee. FMCSA does not charge for DataQs.
4. Your part. You send the records TSH asks for within 5 business days. If you submit from your own DataQs account, you submit within 2 business days and forward every message from FMCSA or the state within 2 business days. TSH is not responsible for a result caused by late or missing records. You confirm you may share your driver's logs, reports and records with TSH for this request.
5. No guaranteed result. Inspection requests are decided by the state agency that did the inspection. Crash requests are decided by FMCSA. In both, the requester has to prove the case. TSH cannot promise that any violation will be removed or that any crash will be ruled Not Preventable. A crash ruled Not Preventable stays listed on your record with that mark and is taken out of your Crash BASIC score; it is not deleted. The fee pays for the work, not for an outcome.
6. Truthful filing only. The documents and facts you give TSH must be true, complete and unaltered. TSH will not file anything it believes is false. If your records show the violation was correct or the crash was preventable, or something you provided turns out not to be true, TSH will not file (or will withdraw the request) and will tell you why.
7. No refunds. TSH charges the fee only after it reviews your case and accepts it. If TSH does not accept the case, you are not charged. Once paid, the fee is not refundable for any reason, whatever the decision, including if TSH does not file or withdraws the request under section 6.
8. Timing. TSH files within 2 business days after it has your payment and all requested records. For inspections, states must open a request within 7 days and decide the first stage within 21 days; Reconsideration takes up to 21 more days and Final Review up to 45. Crash requests are reviewed by FMCSA, usually in about 3 months; TSH does not control that timeline.
9. Privacy. TSH uses your records only for this request, shares them only with FMCSA and the reviewing state agency, and will not name you publicly without your written permission.
10. Limits. TSH is not a law firm and does not give legal advice or appear in court. A DataQs decision does not change any ticket, citation or insurance claim, and does not decide who is liable for a crash. This agreement does not cover your own duties, such as repairing defects and returning the certified inspection report within 15 days. TSH's total liability under this agreement is limited to the fees you paid under it, and TSH is not liable for lost loads, insurance costs or other indirect losses.
11. Billing questions. If you think a charge is wrong, email info@truckservicehub.us first and give TSH 10 days to fix it before disputing it with your bank or card issuer.
12. General. Wyoming law applies. This is the full agreement for this one inspection or crash, and changes must be in writing (email counts). If one part is invalid, the rest still applies. The person signing confirms they can sign for the carrier. Electronic signatures and checkbox acceptance count as originals.
